Wisuda upāsaka/upāsikā

Venerable Sir, I request to Bhagava, Eventhough He had passed away for a long time, with Dhamma and Sangha as my refuge.
May Venerable Sir know, that since today until till the end of time,
I am Upasaka(Upasika), that have received Triple Gem as my leader.
